We work with you and your care team to get the services you need in the comfort of your home, paid for by your insurance plan. We’ll walk you through the process and get you started as soon as possible — because living your best life shouldn’t have to wait another minute.
Home-based therapy is covered under Medicare and most other insurances. Call to see if your insurance plan qualifies you for at-home PT, OT, and SLP services.
The timeline depends on insurance authorization and therapist availability. Services typically take anywhere from 1-3 weeks to begin.
Unfortunately, insurance will only cover therapy from one provider. A patient cannot receive the same type of therapy from two different providers (ex. PT in a clinic and PT at home).
All therapy requires a doctor's referral and the Plan of Care to be signed. (Paradigm Rehab can provide a doctor via telehealth when needed.)

Name, DOB, Medicare number, Medicaid number (if applicable), Phone number, Address.
